Amara Hospice believes obtaining information about hospice care early on enables families to reduce the stress of last minute decision-making. Although end-of-life care is difficult to discuss, it is best for the patient and family to share their wishes weeks or even months before hospice care becomes necessary. This allows the patient to make informed decisions with the advice and input of loved ones. Once the patient receives an end-of-life diagnosis, the family should be able to focus their time and energy on being with their loved one, rather than on administrative tasks.
After the patient’s physician makes an end-of-life diagnosis, hospice care can begin. The patient, patient’s family, physician, social worker or caseworker may request hospice care from Amara. An Amara Hospice representative visits the patient to conduct a patient assessment within hours of receiving the doctor’s referral. Amara strives to quickly address the patient’s needs and to reduce the stress of waiting, both on the family and patient. The assessment visit is scheduled to suit the needs of the patient and the primary family caregiver.
Every Amara Hospice patient receives care from a skilled hospice team based on directives of the Amara Interdisciplinary Care Plan. Each Interdisciplinary Care Team includes a Registered Nurse (RN), a Licensed Vocational Nurse (LVN), a chaplain, social worker, hospice aides (CNA), a physician on call 24/7 and Amara Hospice volunteers. The Amara Interdisciplinary Team writes a care plan for the patient and family designed to ensure that both receive the care they need. In special cases, the Amara Team can provide dietary counseling, physical or occupational therapy.
Amara Hospice Care is on-call seven days a week, 24 hours a day, including holidays.
